Moerdijk and dementia

The number of residents with dementia in Moerdijk is expected to rise to nearly 1,000 by 2030. However, these figures also mean that an even much larger number of family members, friends and neighbors are also experiencing or will experience the effects of dementia. Municipality of Moerdijk itself is already working in a number of ways to help residents with dementia wherever possible. For example, in cooperation with Alzheimer Nederland, CZ health insurer, Thebe and Surplus, with each organization offering help based on its own strengths. But free training sessions for family caregivers have also been provided: ‘How do I deal with dementia?’ On the Surplus website is more information about dementia. 

Case managers dementia in the municipality of Moerdijk
The dementia case manager specializes in dementia in the elderly. He or she helps seniors with dementia and the people who care for and accompany them (family caregivers). The dementia case manager:

• Provides information on the symptoms of (incipient) dementia;
• supports in arriving at an early diagnosis;
• Provides practical advice on dealing with dementia;
• represents the interests of the elderly person with dementia and their family;
• Establishes a plan of care with those involved;
• refers to appropriate agencies;
• gives elderly people with dementia and family caregivers (emotional) support in coping with dementia.

The Alzheimer's Café
The Alzheimer Café is a meeting place for people with dementia, their partners, families, caregivers, social workers and other interested parties. During bi-monthly meetings of the Alzheimer Café you can learn more about dementia, ask questions of experts and meet fellow sufferers. Each Alzheimer Café focuses on one theme. The Alzheimer Café takes place in the Library in Zevenbergen. The program starts each time at 19.30 hours and lasts until approximately 21.00 hours. You are welcome from 19.00 hours.
